Are There any Yes's on Your Medical?

If there are any yes's on this form, for your own safety, we ask that you visit a physician to get authorization to dive. This form is very important; if you show up to the pool with a yes, and do not have a signature from a physician, you will not be able to dive.

Are You Under 18?

If you are under 18 we also require signatures from your parent or guardian. If a parent or guardian is unable to join you on the first night of your course, the following forms must all be downloaded, printed, signed by you and your parent/guardian, and brought to your first pool session. This paperwork is required in order to begin any in-water activities. Failure to bring these forms may result in additional fees incurred due to pool booking and rescheduling.
